Deal with them before they become a problem

Wisdom teeth are the third molars, and they usually try to arrive between about seventeen and twenty-five. Many jaws no longer have room for them. When there is not enough space, the tooth becomes impacted: stuck against the tooth in front, angled sideways, or trapped in bone.

An impacted wisdom tooth that is partly through the gum is the common troublemaker. The flap of tissue over it traps food and bacteria, and the resulting infection, called pericoronitis, is painful and recurs. It can also cause decay on the back of the perfectly healthy molar in front of it.

Not every wisdom tooth needs to come out. If they are fully erupted, in a sensible position, cleanable, and not causing problems, leaving them alone is a legitimate plan. We monitor with imaging and make the call on evidence rather than routine.

Why remove them in the teens or twenties?

The roots are not fully formed yet

Younger patients have shorter, less developed roots and softer bone. Removal is simpler and healing is typically faster and less eventful.

It protects the molar in front

An angled wisdom tooth pressing against the second molar causes decay and bone loss on a healthy tooth that is far more valuable than the one causing the damage.

Planned beats urgent

Scheduled surgery during a school break is a different experience from an emergency at nineteen with a swollen face during exams.

Recurring infections stop

Pericoronitis around a partly erupted wisdom tooth tends to come back repeatedly until the tooth is removed.

Cysts are rare but real

A tooth retained in bone can develop a cyst around it that damages surrounding bone. Imaging is how we catch that before it matters.

How we handle wisdom teeth

  1. Monitoring

    From the mid-teens we track their development with imaging at routine checkups. That is how we see the angle they are heading in before symptoms start.

  2. Assessment

    Dr. Petti reviews the imaging, including the relationship to the nerve in the lower jaw and the sinus above the uppers. Some cases need 3D imaging to see that clearly.

  3. The recommendation

    If they should come out, we explain why, which ones, and what anesthesia suits. If they can stay, we say that too and keep watching. You get a written estimate before booking.

  4. Surgery

    Local anesthetic, with sedation if chosen. Impacted teeth may need a small incision and sometimes sectioning to remove cleanly. Most cases take under an hour, often much less.

  5. Recovery

    Gauze pressure, ice for the first day, soft food, and no straws or smoking. Swelling peaks around day two. Most people take two to three days off, more if all four came out at once.

If you’re considering wisdom teeth removal, it’s normal to have questions. Here’s what patients ask Dr. Dunn most often.

Does everyone need their wisdom teeth out?

No. If they come in fully, sit in a sensible position, can be cleaned, and are not damaging anything, they can stay. We recommend removal based on imaging and symptoms, not on a rule.

What is the best age?

Late teens to early twenties tends to be easiest, because the roots are not fully developed and the bone is more forgiving. That said, plenty of adults have them removed successfully later. It is simply a bigger recovery.

Can all four come out at once?

Usually, yes, and most people prefer one recovery to four. With sedation it is a single appointment. If you would rather split them, that is fine too.

Will I be asleep?

You can be. Options run from local anesthetic alone through to general anesthesia, and having a surgeon on site is what makes the deeper options possible here. See sedation and general anesthesia.

How long is recovery?

Most people take two to three days before feeling reasonably normal, with swelling peaking around day two. Full healing of the sockets takes a few weeks. Impacted teeth mean a longer recovery than erupted ones.

How bad is the pain?

Manageable for most people with what we prescribe or recommend, worst on days one and two. Ice in the first twenty-four hours makes a real difference to both swelling and comfort. Pain that gets worse after day three rather than better is worth a call.

What is dry socket and how do I avoid it?

Loss of the protective blood clot from the socket, exposing bone. It is painful and typically appears three to five days after surgery. Avoid straws, smoking, spitting, and vigorous rinsing for the first several days. If it happens, call us. Treatment brings quick relief.

What about nerve damage?

Lower wisdom teeth can sit close to the nerve that supplies feeling to the lip and chin. Temporary numbness is uncommon and permanent change is rare. This is exactly what the imaging assesses beforehand, and it is part of why the position of the roots matters so much to the plan.

What can I eat afterwards?

Soft, cool foods for the first day or two: yogurt, smoothies eaten with a spoon rather than a straw, eggs, mashed potatoes. Avoid seeds, rice, and anything crunchy that can lodge in the sockets.

When can I go back to school or work?

Most people manage after two to three days. If you had all four removed, or they were deeply impacted, give yourself the rest of the week. Avoid heavy exercise for several days as it can restart bleeding.

Will insurance cover it?

Many dental plans cover a portion, and surgical removal of impacted teeth is sometimes covered partly under medical plans. We verify benefits and give you a written estimate before surgery, with financing available for the balance.
